TRAVEL AUTHORIZATION TERMS & CONDITIONS
The travel authorization (certificate) is valid for flights served by Alaska Airlines flights numbered (1-999, 2000 -2999) and/or Sky West Airlines flights (3450-3499)
Mileage Plan Miles cannot be earned or credited. First Class paid upgrades or mileage plan upgrades are allowed. Connections are also allowed (subject to fare rules) Standard baggage fees apply; see our checked baggage policy and flight schedules at www.alaskaair.com for more details. Individual travel taxes may apply. Once ticketed, fare rules apply regarding changes to itinerary. The certificate is valid for one year from date of issue with no extensions permitted. The following black-out dates apply: NOV 21-NOV 28, 2016 | DEC 18, 2015- JAN 2, 2017
The certificate is void if bartered or sold. If lost, the certificate may not be reissued.
Reservations and Ticketing Instructions Included on Certificate:
- Allow two (2) weeks from the time of reservation and your flight. To make your reservation, call the Alaska Airlines Commercial Desk (M-F 8am-11pm MST, Sat/Sun 9am-7pm MST) at 1-888-327-2755. Advise the reservations agent that you have a Travel Authorization Form along with any inquiries you may have regarding MVP, Gold or 75K Gold Membership benefits.
- When you have confirmed your flight(s), ask the agent for your confirmation code. Complete the bottom portion of this letter then mail the entire letter to:
